Heterotopic pregnancy is defined as a simultaneously intra- and extrauterine pregnancy. The condition is exceedingly rare, with an estimated incidence of one in 30,000 though more frequent in the presence of risk factors. In this case report, a 29-year-old woman, nine weeks pregnant, presented in the emergency department with lower abdominal pain worsening throughout the day. Ultrasound showed an intrauterine pregnancy and excess fluid in the pelvis. Laparoscopy revealed a tubal pregnancy and a salpingectomy was performed. The patient was discharged the following day with ongoing intrauterine pregnancy.
