Protection of Transcatheter Mitral Valve Replacement During Valve-in-Valve Transcatheter Aortic Valve Replacement

A 72-year-old woman with prior transcatheter aortic valve replacement (TAVR) with a 23 mm SAPIEN 3 (Edwards Lifesciences), transcatheter mitral valve replacement (TMVR) with a 29 mm SAPIEN 3 Ultra who presented with heart failure and was found to severe bioprosthetic aortic stenosis of the TAVR valve. Computed tomography revealed that the distal aspects of the TMVR and TAVR were closely abutting. Therefore, a 30 mm balloon was placed in the left atrium for TMVR modification during the procedure. TMVR protection should be considered in valve-in-valve TAVRs to ensure that there is no TMVR stent valve deformation after TAVR deployment.
