This 12-year follow-up of a prenatally misdiagnosed subdiaphragmatic ectopic kidney highlights the challenge of distinguishing renal agenesis from anatomical variants. Conservative management proved effective without surgical intervention. Awareness of such anomalies can prevent misdiagnosis, particularly in thoracic imaging, and guide appropriate long-term surveillance through childhood and adolescence.
