This study found that Live Combined Bifidobacterium and Lactobacillus Tablets may lower the risk of overt hepatic encephalopathy (OHE) after transjugular intrahepatic portosystemic shunt (TIPS) procedures. Probiotic use also improved cognitive function in cirrhosis patients.
Area of Science:
- Hepatology
- Gastroenterology
- Microbiome Research
Background:
- Hepatic encephalopathy (HE) is a common complication following transjugular intrahepatic portosystemic shunt (TIPS) in cirrhosis patients.
- Intestinal dysbiosis is strongly linked to HE development.
- The prophylactic role of probiotics in preventing overt HE (OHE) post-TIPS remains unclear.
Purpose of the Study:
- To investigate the efficacy of Live Combined Bifidobacterium and Lactobacillus Tablets in reducing the incidence of OHE after TIPS.
- To evaluate the impact of probiotics on cognitive function in patients undergoing TIPS.
Main Methods:
- An open-label, randomized, placebo-controlled trial involving 142 cirrhosis patients receiving TIPS.
- Participants were assigned to receive either probiotics or a placebo.
- Primary endpoint: incidence of OHE within 24 weeks post-TIPS; Secondary endpoints: cognitive function tests (NCT-A, DST) and other complications.
Main Results:
- The incidence of OHE was significantly lower in the probiotic group (36.1%) compared to the control group (51.4%).
- In patients without OHE, liver transplantation, or death, probiotics significantly improved scores on the Digit Symbol Test (DST) and Number Connection Test-A (NCT-A).
- No significant differences were observed in other cirrhosis-related complications between groups.
Conclusions:
- Live Combined Bifidobacterium and Lactobacillus Tablets may serve as a preventative measure against OHE in cirrhosis patients undergoing TIPS for variceal bleeding.
- Probiotic administration demonstrated improvements in cognitive function in this patient cohort.
- Further multicenter studies are recommended to validate these findings.
