Exploration of the Ejaculatory Anatomy Concept: The EJAC Study

The EJAC study used real-time imaging (transrectal ultrasound and urethroscopy) to investigate the biomechanics of ejaculation in three healthy men. The results reveal a complex, multistage process that challenges traditional models of ejaculation. Key findings include observation of the creation of a peri-verumontanum antechamber where semen accumulates during the emission phase. During expulsion, we observed peri-verumontanum contractions, an anterior tilt movement of the verumontanum, and coordinated peri-urethral contractions. The tilting action of the verumontanum appears to be decisive in directing antegrade semen flow. Our findings provide novel insights into the biomechanics of ejaculation, especially regarding the role of the peri-verimontanum area, and may inform future treatments for benign prostatic obstruction aimed at preserving sexual function.
