    Summary

    Mini-implants for mandibular overdentures showed greater bone loss compared to conventional implants. Locator attachments on conventional implants offered the best radiographic outcomes, maintaining bone levels effectively.

    Background:

    • Complete edentulism affects denture stability and patient satisfaction.
    • Conventional implants offer robust retention but require adequate bone volume.
    • Mini-implants provide a less invasive alternative for denture stabilization.

    Purpose of the Study:

    • To compare radiographic outcomes of mandibular overdentures retained by mini-implants versus conventional implants.
    • To evaluate different attachment systems (locator and ball-and-socket) on conventional implants.
    • To assess marginal bone loss around implants over 12 months.

    Main Methods:

    • Randomized controlled trial with 21 edentulous patients.
    • Three groups: mini-implants (n=4), conventional implants with locator attachments (n=2), conventional implants with ball-and-socket attachments (n=2).
    • Cone-beam computed tomography (CBCT) assessed peri-implant bone levels at baseline, 6, and 12 months.

    Main Results:

    • All groups experienced bone loss over 12 months.
    • Mini-implant group showed significantly higher bone resorption (0.81±0.11 mm) than conventional implant groups (0.41–0.45 mm).
    • No significant difference in bone loss between locator and ball-and-socket attachments on conventional implants.

    Conclusions:

    • Mini-implants resulted in greater marginal bone loss compared to conventional implants.
    • Conventional implants with locator attachments demonstrated the most favorable radiographic outcomes.
    • All tested systems maintained bone within acceptable clinical limits.
