The issue of concomitant autoimmune disorders is an acute problem in neurology, with a diversity of conditions co-existing, and hampering diagnosis or prompt therapeutic interventions. We discuss the case of a middle age Caucasian male suffering from ulcerative colitis of a severity warranting surgery and pharmacological therapy, which has been complicated with neurological symptomatology. The co-existence of ulcerative colitis and myasthenia gravis might be a rarity, although the autoimmune background of both occurrences should lead the clinicians to considering such a setting, and treat accordingly both diseases. Unexplained cranial nerve involvement should be regarded as a red flag, necessitating neurological consultancy and appropriate intervention among patients with inflammatory bowel diseases.
