Epidemiology of Deaths and Injuries in Special Operations Forces: A Systematic Review

Background:
Advances in war injury epidemiology have significantly improved combat casualty care. Innovations in medical and surgical strategies, evacuation protocols, and protective equipment have contributed to a reduction in preventable deaths. However, Special Operations Forces (SOF) continue to face unique challenges, often operating in austere environments. Special Operations Forces missions require specific training and tailored medical strategies. This systematic review hypothesizes that injury patterns among SOF personnel during combat differ from those observed in conventional forces, synthesizing current data on SOF injury epidemiology.
Materials And Methods:
The MEDLINE database was searched for studies on combat and non-combat injuries among SOF populations up to November 1, 2023. The review followed Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, and the protocol was registered with PROSPERO. Case reports, letters to the editor, and conference abstracts were excluded.
Results:
This review included 16 observational studies published between 2000 and 2023, selected from 1,423 records. Of the included studies, 14 were retrospective studies, 1 were cohort studies, and 1 was a systematic review. The data encompassed 853 deaths, 1,327 combat injuries, primarily involving U.S. SOF personnel. Most studies focused on injuries sustained in Afghanistan, Iraq. Explosions were the leading cause of combat injuries and deaths, followed by gunshot wounds and blunt trauma. Catastrophic tissue destruction was the primary mechanism of fatal injuries. Special Operations Forces personnel exhibited higher rates of gunshot wounds, while conventional forces were more frequently exposed to blast injuries. Special Operations Forces injuries were also found to be more severe reflecting their unique tactical risks. Some studies shared overlapping datasets, which may influence the findings.
Conclusions:
This systematic review highlights SOF-specific injury patterns and underscores the critical role of far-forward combat casualty care in reducing SOF combat mortality.
