Summary

Scanning single-molecule counting (SSMC) of miR-200c shows promise as a biomarker for detecting lymph node metastasis in lung cancer. This rapid molecular assay complements traditional methods for improved diagnostic accuracy.

Background:

  • Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is standard for mediastinal lymph node sampling.
  • Cytopathological diagnosis can be limited by insufficient atypical tumor cells.
  • Scanning single-molecule counting (SSMC) offers rapid molecular expression analysis.

Purpose of the Study:

  • To assess the correlation between miR-200c expression measured by SSMC and lymph node metastasis.
  • To evaluate SSMC as a rapid diagnostic tool for molecular nodal staging.

Main Methods:

  • MicroRNA (miRNA) was extracted from lavage fluid following EBUS-TBNA in lung cancer patients.
  • miR-200c expression was measured using SSMC and compared with quantitative real-time polymerase chain reaction (RT-qPCR).

Main Results:

  • SSMC demonstrated a strong positive correlation with RT-qPCR for miR-200c detection (R² = 0.81, p < 0.0001).
  • High miR-200c expression detected lymph node metastasis with 85.7% sensitivity and 83.3% specificity (AUC = 0.88).
  • Rapid SSMC results were obtained within 30 minutes, with no discordance observed.

Conclusions:

  • miR-200c expression measured by SSMC can serve as a molecular biomarker for nodal staging.
  • Rapid SSMC analysis offers a significant time advantage over conventional methods.
  • SSMC may enhance rapid on-site cytological evaluation for lung cancer diagnosis.
