Background:

Prostate cancer treatment often involves radiation therapy.
SpaceOAR hydrogel is used to reduce rectal toxicity during prostate radiotherapy.
Complications associated with SpaceOAR implantation are rare but can occur.

Purpose of the Study:

To report a case of prostatic abscess formation following SpaceOAR hydrogel implantation for prostate cancer radiotherapy.
To discuss potential causes and management of this rare complication.

Main Methods:

A 79-year-old male patient received external-beam radiation therapy with hormone therapy for prostate cancer.
SpaceOAR hydrogel was implanted prior to stereotactic radiotherapy (Cyberknife).
Diagnosis of prostatic abscess was confirmed via clinical, CT, and MRI findings.

Main Results:

The patient developed a fever and was diagnosed with a prostatic abscess at the SpaceOAR insertion site post-radiotherapy.
Initial antibiotic treatment was ineffective.
Transurethral resection and vesicostomy for abscess drainage resulted in rapid improvement.

Conclusions:

SpaceOAR hydrogel may have inadvertently penetrated the prostatic capsule during implantation.
Urinary retention or catheterization could have contributed to a urinary tract infection and subsequent abscess formation.
Prompt drainage is crucial for managing prostatic abscesses in patients treated with SpaceOAR.
