Abstract:
Necrotizing fasciitis is a rapidly progressive and life-threatening soft tissue infection usually caused by bacteria, with a high mortality of up to 80% when untreated. Any delay in time further increases the risk of mortality. Thus, early recognition and treatment are of upmost importance. The clinical assessment, consideration of laboratory results and surgical exploration are important diagnostic procedures. Cross-sectional imaging is highly sensitive with respect to assessment of the extent (computed tomography) and differential diagnostic considerations (magnetic resonance imaging). In individual cases imaging can provide valuable and specific evidence of necrotizing fasciitis, such as deep fascial involvement; however, a major problem arises when diagnostic imaging would lead to a critical delay of urgently needed surgical procedures, but diagnostic uncertainties still exist. For these reasons the diagnostic work-up in necrotizing fasciitis inherently needs a thorough interdisciplinary discussion among experienced specialists.
