Summary

Heterophile antibodies frequently interfere with clinical immunoassays, leading to inaccurate results. Heterophile blocking tubes (HBT) effectively mitigate this interference, improving diagnostic accuracy for common viral infections.

Area of Science:

  • Clinical Chemistry
  • Immunology
  • Diagnostic Microbiology

Background:

  • Immunoassays are crucial for clinical diagnostics but are prone to interference.
  • Heterophile antibodies are a common source of interference in immunoassay testing.
  • Accurate detection of viral IgM antibodies is vital for diagnosing acute infections.

Purpose of the Study:

  • To assess heterophile antibody interference in routine clinical IgM assays.
  • To evaluate the efficacy of heterophile blocking tubes (HBT) in reducing interference.
  • To determine the impact of HBT on the clinical interpretation of viral serology results.

Main Methods:

  • Collected 185 residual serum samples with positive or equivocal IgM results for common viruses.
  • Tested samples for Epstein-Barr virus (EBV), herpes simplex virus (HSV), and other viral infections.
  • Pretreated samples with HBT and reanalyzed, comparing results with untreated samples.

Main Results:

  • HBT pretreatment significantly reduced reactivity levels for EBV VCA and HSV.
  • Positivity rates for EBV VCA and HSV were substantially decreased after HBT treatment.
  • Clinical interpretation of EBV status was altered, reclassifying primary infection diagnoses.

Conclusions:

  • Heterophile antibody interference is prevalent in routine viral IgM testing.
  • HBT pretreatment is effective in eliminating heterophile antibody interference.
  • HBT can improve the reliability of clinical laboratory results and diagnostic accuracy.
