Local Anesthetics: Differential Sensitivity of Nerve Fibers

1.4K
Local anesthetics (LAs) block the sodium channels of nerve trunks, sensory nerve endings, and neuromuscular junctions. Although LAs can block all kinds of nerves, the sensitivity of nerve fibers differs according to nerve types and structures. LAs are known to block myelinated fibers faster than unmyelinated ones. Also, they block pain or sensory neurons at low concentrations without affecting the motor neurons involved in muscle contractions. This helps relieve labor pain without affecting the...

Area of Science:

  • Neurology
  • Clinical Medicine
  • Peripheral Nerve Disorders

Background:

  • Entrapment and impingement neuropathies involve peripheral nerve compression or irritation.
  • These conditions result in sensory and motor deficits affecting a single nerve.

Purpose of the Study:

  • To review the pathophysiology, clinical presentations, and evidence-based management of common entrapment neuropathies.
  • To discuss less common nerve impingement disorders, their clinical relevance, and diagnostic difficulties.
  • To explore the association between systemic diseases and entrapment neuropathies.

Main Methods:

  • Literature review of pathophysiology, clinical presentations, and management strategies.
  • Discussion of common and less common nerve entrapment syndromes.
  • Exploration of systemic disease associations.

Main Results:

  • Detailed discussion of carpal tunnel syndrome, ulnar neuropathy at the elbow, fibular neuropathy, radial nerve entrapment, and lateral femoral cutaneous nerve entrapment.
  • Highlights clinical relevance and diagnostic challenges of less common neuropathies.
  • Examines systemic diseases frequently associated with entrapment neuropathies.

Conclusions:

  • Comprehensive understanding of entrapment neuropathies is crucial for accurate diagnosis and effective management.
  • Timely identification and treatment can mitigate sensory and motor deficits.
  • Awareness of systemic associations aids in holistic patient care.
