Neck hematoma after parathyroidectomy

Background:
The purpose of this study was to determine the rate of neck hematoma requiring urgent evacuation following parathyroidectomy.
Methods:
The rate of neck hematoma was examined for all patients who underwent parathyroidectomy from 1992 to 2025, and a comparative analysis was completed for patients with primary hyperparathyroidism (pHPT) versus patients with renal HPT.
Results:
1002 consecutive patients underwent parathyroidectomy; 922 with pHPT and 80 with renal HPT. Six (0.6 %) patients, all with pHPT, developed neck hematoma requiring urgent evacuation a median of 32 h (IQR 1.48-68.7) after parathyroidectomy. No patient with renal HPT developed a neck hematoma (χ2 = 0.48, p = 0.49). Median ASA score for patients with neck hematoma was 3 (IQR 2.5-3) versus 2 (IQR 2-3) for patients without neck hematoma (p = 0.18) CONCLUSION: The rate of life-threatening neck hematoma following parathyroidectomy is rare and not significantly different in patients with pHPT than patients with renal HPT.
