Older adults are able to live well with pulmonary disease when supported by an experienced and knowledgeable care team. Advance care planning can enhance patients' confidence and sense of control related to their chronic illness. A high-quality conversation between the patient and their surrogate decision maker, facilitated by a clinician who understands the nuances of the options, is the most important aspect of the advance care planning process. The goal is to embrace the inherent uncertainties, and align care with the patient's values and preferences, so that patients and their care takers can focus on what matters most to them.
