This case report covers the novel use of an Nd:YAG Ultra-Q Reflex laser to treat a symptomatic vitreous cyst in a 47-year-old man. Following a single 0.3-mJ laser pulse applied to the cyst's superior pole, controlled fluid egress led to cyst collapse and inferior displacement without complications. The patient experienced immediate visual improvement, with the cyst remaining inactive in the inferior vitreous at 1-month follow-up and demonstrating no significant change in size when located in the anterior vitreous at 10 months. This laser cystotomy technique presents a safe, effective, and minimally invasive alternative to traditional management.
