Background:

Purpose of the Study:

  • To compare the incidence of recurrent patellar instability and the need for subsequent stabilization procedures in adolescents with and without OCF after a first-time patellar dislocation.
  • To evaluate clinical outcomes, including return to sport, in these patient groups.
  • To identify factors influencing recurrent instability in adolescent patellar dislocations.

Main Methods:

  • Retrospective cohort study of 33 adolescents with first-time patellar dislocation (2016-2021).
  • Patients were divided into OCF (n=15) and non-OCF (n=18) groups, with a minimum 3-year follow-up.
  • Primary outcomes: recurrent instability and need for stabilization; secondary outcomes: clinical scores, ROM, and return to sport.

  • The OCF group showed a significantly higher incidence of recurrent patellar instability (66.7% vs 27.8%) and greater need for stabilization surgery (53.3% vs 16.7%) compared to the non-OCF group.
  • A higher tibial tuberosity-trochlear groove (TT-TG) distance was observed in the OCF group (17.1 mm vs 14.5 mm).
  • No significant differences were found in clinical outcome scores, range of motion, or return to sport rates between the groups.

Conclusions:

  • Adolescents with first-time patellar dislocation and OCF experience significantly higher rates of recurrent instability and require more stabilization procedures.
  • Elevated TT-TG values are associated with increased instability in this population.
  • Targeted interventions are recommended for adolescents with OCF and high-risk anatomical factors to mitigate recurrent patellar instability.
