Abstract:
Facial palsy causes significant functional and social impairments, affecting facial movement, expression, and quality of life. In long-standing cases-lasting over 18 months-spontaneous recovery is unlikely, and dynamic reconstruction is needed to restore facial symmetry and a natural smile. Labbé technique, a regional temporalis muscle transfer, is a well-established method that offers good static and functional results with a single surgical stage. However, the classic approach involves osteotomy of the zygomatic arch and detachment of the coronoid process, which can lead to complications such as tendon devascularization or dehiscence and facial contour deformities. This article describes a modification of the Labbé technique that avoids zygomatic arch osteotomy, preserves the coronoid process attached to the tendon, and secures muscle reattachment with screws in the temporal fossa. These changes reduce surgical risks while maintaining functional outcomes, offering an effective option for smile reanimation in long-standing facial palsy.
