Acromegalic cardiomyopathy is one of the common comorbidities of cardiovascular diseases in patients with acromegaly, and it is also a major risk factor affecting the survival rate of patients with acromegaly. The perioperative management of such patients is a challenge for anesthesiologists. In this case report, we describe a patient with acromegalic cardiomyopathy who successfully underwent transsphenoidal endoscopic resection of a pituitary lesion under general anesthesia. For the perioperative anesthesia management of patients with acromegalic cardiomyopathy, it is crucial to closely monitor their high airway risk and compensatory cardiac function challenges. By comprehending the pathophysiological mechanisms of the disease and conducting thorough preoperative diagnostic evaluations, implementing individualized strategies and fostering multidisciplinary collaboration can effectively enhance the perioperative safety of these patients.
