Thyroid dysfunction is linked to critical illness polyneuropathy (CIP) in ICU patients. Routine thyroid screening may help identify patients at risk for this neuromuscular complication.

- To investigate the role of thyroid dysfunction in CIP pathogenesis.

- Nine out of 24 analyzed patients (37.5%) developed new or progressive CIP by day seven.
- Thyroid dysfunction was the only clinical variable significantly associated with CIP development (p = 0.026).
- 32% of CIP patients had underlying hypothyroidism or hyperthyroidism.

- Thyroid dysfunction is significantly associated with CIP development in critically ill patients.
- Thyroid status may play an underrecognized role in CIP pathogenesis.
- Routine thyroid screening in ICU patients may be beneficial.
