Purpose of the Study:
- To investigate if guiding endoscopists' gaze toward the periphery of the screen enhances adenoma detection during colonoscopy.

- Peripheral gaze rate increased in the intervention group (33.7% vs. 25.9%, P<0.001) without increasing observation time.
Conclusions:
- Real-time guidance to increase peripheral gaze significantly improved adenoma and polyp detection.
- This approach is low-burden, scalable, and effective for enhancing colonoscopy quality without prolonging procedure time.
