    Poly-L-lactic acid (PLLA) microspheres provide durable correction for nasolabial folds up to 48 weeks, outperforming hyaluronic acid (HA) despite requiring more initial treatments. Both PLLA and HA fillers demonstrated similar safety profiles in this trial.

    Background:

    • Nasolabial folds (NLFs) correction is a key concern in aesthetic medicine.
    • Poly-L-lactic acid (PLLA) microspheres offer potential for longer-lasting results than hyaluronic acid (HA) due to their biostimulatory properties.

    Purpose of the Study:

    • To evaluate the efficacy and safety of PLLA microspheres compared to HA for correcting moderate-to-severe NLFs.
    • To compare the durability and patient outcomes of PLLA versus HA fillers.

    Main Methods:

    • A multicenter, randomized, double-blind, parallel-controlled trial involving 252 participants in China.
    • Participants received either PLLA or HA filler, with outcomes assessed using the Wrinkle Severity Rating Scale (WSRS) and Global Aesthetic Improvement Scale (GAIS) at 48 weeks.

    Main Results:

    • PLLA showed a significantly higher effective correction ratio than HA at 48 weeks (92.4% vs 59.3%).
    • While HA provided immediate effects, PLLA demonstrated superior and sustained improvement from week 36 onwards.
    • Adverse events were mild and transient for both PLLA and HA fillers.

    Conclusions:

    • PLLA requires more initial treatments but offers more durable correction up to 48 weeks compared to HA.
    • HA provides rapid improvement, whereas PLLA offers longer-lasting results with similar safety.
    • Individual patient priorities should guide the selection between PLLA and HA fillers.
