Design and Implementation of Novel Internal Mini Fellowship Concentrations Within a Radiation Oncology Residency

Purpose:
Resident and faculty impressions of novel mini fellowship concentrations in multiple disease sites during radiation oncology training were assessed.
Methods And Materials:
In 2022, mini fellowship concentrations in gynecologic brachytherapy, central nervous system, head and neck, and gastrointestinal radiation oncology were established. Concentration curricula included completion of procedures and treatment planning cases in addition to research activity. Residents completed their concentrations during their scholarly time, typically during their third year of radiation oncology training. Concentrations were voluntary, and residents were not responsible for other clinical duties while completing their concentrations (although some did opt to be in clinic and saw consults). Surveys were distributed to residents, recent graduates, and faculty concentration directors.
Results:
Surveys were distributed to 19 current and former residents with a 95% (n = 18) response rate. Of respondents, 16 were interested in or had already completed a concentration. Two respondents indicated that a concentration would detract from their research time, reflecting the proportion of residents enrolled on the research scholar track/Holman pathway. Residents recorded significant case volume during their concentrations, approaching Accreditation Council for Graduate Medical Education-required total case volume. Of the 9 respondents who were currently completing or had completed a concentration, common motivators for pursuing a concentration were mastery of a disease site/procedure (89%), maintenance of contouring/procedural competency during their scholarly year (67%), and appeal to potential employers (56%). On faculty survey, all respondents indicated that concentrations provided educational benefit and mastery of their disease site.
Conclusions:
Residents and faculty had favorable experiences with mini fellowship concentrations in gynecologic brachytherapy, central nervous system, head and neck, and gastrointestinal radiation oncology. Most residents appreciated the extra case experience, hands-on teaching, and opportunities for research collaboration. These concentrations could serve as a model for other programs.
