Summary

The C-reactive protein (CRP)-albumin-lymphocyte (CALLY) index showed a negative correlation with the Sequential Organ Failure Assessment (SOFA) score in sepsis patients. This novel biomarker demonstrated similar performance in both male and female sepsis cases.

Background:

  • Sepsis and multiorgan dysfunction syndrome (MODS) pose significant global health challenges.
  • The Sequential Organ Failure Assessment (SOFA) score is a standard morbidity assessment tool for critically ill patients.
  • The C-reactive protein (CRP)-albumin-lymphocyte (CALLY) index is an emerging composite biomarker for sepsis diagnosis.

Purpose of the Study:

  • To evaluate the CALLY index in male and female sepsis patients.
  • To correlate the CALLY index with the SOFA score at patient admission.

Main Methods:

  • An observational study conducted at Kalinga Institute of Medical Sciences, India, from August to December 2025.
  • Included adult sepsis patients meeting Sepsis-3 criteria.
  • Recorded absolute lymphocyte count, CRP, and serum albumin for CALLY index calculation; SOFA scores were noted on days 1, 3, and 7. Spearman correlation assessed the association between SOFA score (day 1) and CALLY index.

Main Results:

  • Analyzed 263 sepsis patients (median age 62.0 years; 61.22% male).
  • Median CALLY index was 2.48 (1.34-4.94); median SOFA score at admission was 5.0 (3.0-8.0).

  • Study limitations include small sample size and missing data on various clinical parameters, restricting generalizability.
