Insights
A rare case of hydatid cyst disease involving the right atrium (RA) and liver presented with severe symptoms. Successful surgical removal of the RA cyst and pulmonary embolism treatment were achieved.
Area of Science:
- Medicine
- Parasitology
- Cardiology
Background:
- Hydatid cyst disease, caused by Echinococcus granulosus, typically affects the liver and lungs.
- Cardiac involvement, particularly in the right atrium (RA), is exceptionally rare.
Purpose of the Study:
- To report a unique case of right atrial hydatid cyst presenting with massive hemoptysis and pulmonary embolism.
- To highlight the diagnostic and therapeutic challenges of rare cardiac echinococcosis.
Main Methods:
- Transthoracic echocardiography to visualize the RA mass.
- Chest computed tomography angiography for multiorgan assessment.
- Serological testing for echinococcosis confirmation.
- Surgical excision of the RA cyst and pulmonary embolectomy under cardiopulmonary bypass.
Main Results:
- A large, multicystic mass was identified in the RA, suggestive of a hydatid cyst.
- Pulmonary cystic lesions, pulmonary embolism, and a hepatic hydatid cyst were confirmed.
- The patient underwent successful surgical intervention with complete cyst excision and embolectomy.
Conclusions:
- Right atrial hydatid cyst is a rare but critical condition that can lead to life-threatening complications like pulmonary embolism.
- Comprehensive imaging and prompt surgical management are essential for successful outcomes in complex echinococcosis cases.
- This case underscores the importance of considering rare diagnoses in patients with unexplained symptoms and multiorgan involvement.
Abstract:
Hydatid cyst disease, caused by Echinococcus granulosus, primarily affects the liver and lungs, while cardiac involvement, especially the right atrium (RA), is extremely rare. We report the case of a 21-year-old woman who presented with massive haemoptysis and hypoxemia. Transthoracic echocardiography demonstrated a large multicystic mass occupying the RA cavity, adherent to the atrial wall and interatrial septum, strongly suggestive of a hydatid cyst. Chest computed tomography angiography confirmed multiple pulmonary cystic lesions, intravascular filling defects consistent with pulmonary embolism, and a large hepatic hydatid cyst (5 × 4 cm). Serological testing for echinococcosis was positive. The patient underwent successful surgical excision of the RA cyst and pulmonary embolectomy under cardiopulmonary bypass. This case illustrates the rare presentation of an RA hydatid cyst with multiorgan involvement and associated pulmonary embolism, emphasizing the critical role of comprehensive imaging and timely surgical intervention in the management of complex echinococcosis.
