Background:

  • Minimally invasive parathyroidectomy (MIP) relies on accurate preoperative localization of parathyroid adenomas.
  • Current imaging modalities vary in diagnostic accuracy, impacting surgical outcomes.

Purpose of the Study:

  • To compare the diagnostic accuracy of expert-performed ultrasonography (US) against community US and other imaging techniques for preoperative parathyroid adenoma localization.
  • To evaluate the utility of expert US in complex cases, including ectopic adenomas and multiglandular disease.

Main Methods:

  • Retrospective diagnostic accuracy study of 227 patients with primary hyperparathyroidism.
  • Preoperative imaging included expert US, community US, 99mTc-MIBI, SPECT-CT, and 4D-CT.
  • Sensitivity, specificity, and positive predictive value (PPV) were calculated and compared.

Main Results:

  • Expert US demonstrated high sensitivity (89%), specificity (98%), and PPV (95%), accurately localizing adenomas in 89% of cases.

  • Referral to an expert ultrasonographer is recommended for suspected ectopic adenomas, multiglandular disease, or before considering 4D-CT or bilateral neck exploration.
