Static and dynamic pedobarographic evaluation after UCBL orthoses in pediatric patients with foot arch disorder

Abstract:
Pes planus is one of the most common foot deformities in children. Pedobarographic assessment provides objective information on plantar pressure distribution and may be useful for monitoring treatment-related changes. This prospective pretest-posttest study evaluated static and dynamic pedobarographic changes following University of California Biomechanics Laboratory (UCBL) orthosis use in 90 children aged 4-16 years with flexible pes planus. All participants used a UCBL orthosis for 3 months. Static and dynamic pedobarographic measurements were performed at baseline and after treatment. Regional plantar pressure and contact area were analyzed for both feet. After UCBL orthosis use, static assessment demonstrated significant reductions in plantar pressure, predominantly in the midfoot and forefoot regions ( P < 0.05). Significant reductions were also observed in maximal forefoot pressure, plantar force percentages, contact area, and maximal force. Dynamic analysis revealed decreased plantar pressure in the phalanx and medial forefoot bilaterally, with additional reductions in the lateral forefoot and hindfoot on the left side ( P < 0.05). Total plantar contact area during gait was also reduced. UCBL orthosis use results in measurable functional changes in plantar pressure distribution in children with flexible pes planus. Pedobarographic evaluation may serve as a useful tool for monitoring orthotic effectiveness in pediatric foot arch disorders.
