Abstract:
The patient-centered medical home model has become a cornerstone for primary care delivery with over 13,000 recognized practices nationwide. However, the model still faces significant barriers including increases in data and administrative tasks and fragmented electronic health records. In this Perspective, we address advances in artificial intelligence that present opportunities to address these challenges while raising important implementation considerations.
