Association between Proteinuria and Risk of Chronic Inflammatory Demyelinating Polyneuropathy/Multifocal Motor

Proteinuria, or protein in urine, is linked to a higher risk of developing chronic inflammatory demyelinating polyneuropathy (CIDP) and multifocal motor neuropathy (MMN). This suggests potential shared disease pathways requiring further investigation.

Background:

Purpose of the Study:
- To investigate the association between proteinuria and the subsequent incidence of CIDP/MMN.

- A population-based retrospective cohort study was conducted using the Korean National Health Insurance Service database.
- Over 3.9 million individuals aged 20 years and older were followed from 2009 to 2020.
- Proteinuria was assessed by urine dipstick, and CIDP/MMN diagnosis was confirmed using ICD-10 codes and rare disease program codes. Cox proportional hazards models were used for analysis.
Main Results:
- Proteinuria was present in 2.5% of the study population at baseline.
- The incidence rate of CIDP/MMN was significantly higher in individuals with proteinuria (1.44 per 100,000 person-years) compared to those without (0.50 per 100,000 person-years).
- After adjusting for confounders, proteinuria remained a significant risk factor for CIDP/MMN (adjusted hazard ratio 2.32).
Conclusions:
- Proteinuria is associated with a modest increased risk of developing CIDP/MMN.
- Shared immunopathologic mechanisms between kidney and peripheral nerve diseases may exist.
- Consideration of neurologic monitoring in individuals with proteinuria is suggested.
