This review explores the responsible innovation concept in endodontics, emphasising its educational implications, biological accountability in clinical outcomes and a framework to guide everyday practice. Responsible innovation in endodontics must transcend mere technical advancement; it demands a comprehensive critical appraisal of practices, ethical awareness and a stewardship of long-term patient outcomes. Educational frameworks that prioritise reflective practice and encourage practitioners to continuously assess and refine their methodologies are essential. Additionally, a focus on biological plausibility and recognition of delayed identification of unfavourable outcomes is crucial. Failing to incorporate these principles may allow suboptimal innovations to persist within the field, potentially compromising patient safety and treatment effectiveness. Teaching responsible innovation should be a central objective in postgraduate endodontic education. By aligning innovative practices with long-term biological outcomes, ethical standards and societal expectations, the endodontic profession can better maintain its credibility while enhancing the quality of patient care.
