Since early human history, penile size has been associated with perceptions of masculinity and power across cultures. In this context, some individuals have pursued penile enlargement through injection of foreign substances, a practice that, although uncommon, carries significant risk of morbidity. We present a complex case of a male who developed acquired phimosis and obstructive lower urinary tract symptoms (LUTS) following Vaseline injection into the penile shaft, ultimately requiring genital reconstruction.
