Safety and Effectiveness Outcomes of Ab Interno Canaloplasty in Open-angle Glaucoma: 10-year Results

To evaluate long-term outcomes of ab interno canaloplasty in patients with open-angle glaucoma, focusing on intraocular pressure (IOP) control and medication use over a 10-year period.

This retrospective case series evaluated 27 eyes of 22 patients with pseudoexfoliative (PEX) or primary open-angle glaucoma (POAG) who underwent ab interno canaloplasty using the iTrack microcatheter (Nova Eye Medical, Fremont, CA, USA) between October 2014 and October 2016. The majority of procedures were performed in conjunction with cataract surgery (23 of 27 eyes). Main outcome measures were changes in mean IOP and number of glaucoma medications.

Over an average follow-up of 78.7 ± 37.0 months [last follow-up (LFU)], mean IOP (mm Hg) significantly decreased from 19.9 ± 5.2 at baseline to 14.7 ± 3.7 (-26%; p < 0.001) at LFU, while the mean medication use reduced from 1.9 ± 1.0 to 1.0 ± 1.1 (-50%; p = 0.004). Visual field mean deviation (VF MD) (dB) remained stable (-7.86 ± 6.90 to -7.61 ± 6.63; p = 0.498). Among the seven eyes followed for 10 years, IOP decreased from 19.4 ± 3.2 to 14.9 ± 2.3 (-24%, p = 0.010), while medication use remained stable (1.7 ± 0.9 to 2.0 ± 1.2, p = 0.604) with no serious complications or need for additional glaucoma surgery.

iTrack ab interno canaloplasty achieved sustained reductions in IOP and medications, with no safety concerns. Seven eyes were followed up for 10 years, showing sustained IOP reduction.
