A man with post-traumatic epilepsy experienced status epilepticus after stopping antiseizure medications. Brain MRI and EEG showed abnormalities that resolved with treatment, and he remained seizure-free.

Area of Science:

  • Neuroimaging
  • Epilepsy Research
  • Clinical Neurology

Background:

  • Post-traumatic epilepsy (PTE) management requires careful consideration of antiseizure medication (ASM) adherence.
  • Abrupt cessation of ASMs can precipitate refractory seizures, including status epilepticus (SE).
  • Understanding neuroimaging correlates of SE in PTE is crucial for diagnosis and management.

Purpose of the Study:

  • To report a case of status epilepticus in a patient with post-traumatic epilepsy following ASM discontinuation.
  • To illustrate the characteristic neuroimaging findings and their resolution.
  • To highlight the efficacy of lamotrigine monotherapy in seizure control.

Main Methods:

  • Case report of a 41-year-old male patient with PTE.

  • Brain Magnetic Resonance Imaging (MRI) to assess diffusion restriction.
  • Electroencephalography (EEG) to evaluate epileptiform activity.
  • Clinical follow-up and treatment monitoring.
  • Main Results:

    • The patient developed status epilepticus after two months of self-discontinued ASMs.
    • Simultaneous diffusion restriction was observed in the right frontal cortex, corpus callosum, and hippocampus.
    • Concordant right-hemispheric EEG epileptiform activity was detected.
    • All imaging abnormalities resolved completely by day 11.
    • The patient remained seizure-free at three-month follow-up on lamotrigine monotherapy.

    Conclusions:

    • Discontinuation of ASMs can trigger severe seizure exacerbations like SE in PTE patients.
    • Specific patterns of diffusion restriction on MRI correlate with epileptiform activity during SE.
    • Prompt treatment and appropriate ASM selection, such as lamotrigine, are effective in achieving seizure remission and resolving imaging findings.
