Background:

  • Delirium is a significant complication in cardiac surgery patients, linked to adverse outcomes like prolonged ventilation and increased mortality.

Purpose of the Study:

  • To identify clinical factors associated with the occurrence of postoperative delirium in patients undergoing cardiac surgery and admitted to the ICU.
  • To explore potential early markers for postoperative delirium in cardiac surgery patients.

Main Methods:

  • A prospective, observational study conducted in a single cardiothoracic ICU in central Italy.
  • 175 adult patients undergoing cardiac surgery were enrolled and assessed for delirium using the Italian-validated Intensive Care Delirium Screening Checklist (ICDSC) for five days.
  • Univariate and multivariate logistic regression analyses were used to identify associated factors.

Main Results:

  • Delirium occurred in 44.6% of the 175 patients studied.
  • Univariate analysis showed longer mechanical ventilation and higher APACHE II scores were associated with delirium.
  • Multivariable analysis revealed that lower postoperative Glasgow Coma Scale (GCS) scores were independently associated with delirium occurrence (OR = 0.84; 95% CI: 0.71-0.99; p = 0.04).

Conclusions:

  • Delirium is highly prevalent in the cardiac surgery patient population.
  • Lower postoperative GCS scores may serve as an early indicator of neurological vulnerability and delirium risk.
  • Further multicenter research is needed to refine delirium risk stratification and understand the underlying mechanisms of postoperative cognitive dysfunction.
