Late-onset Group B Streptococcus (GBS) disease in infants presents with a wide range of clinical manifestations and can be challenging to diagnose in preterm infants. This vulnerable population is more likely to acquire late-onset GBS disease and has a significantly higher mortality rate compared with term infants. In this article, we discuss triplets born at 28 weeks' gestation who exhibited multiple complications associated with late-onset GBS disease. We also review the epidemiology, clinical presentation, and management of late-onset GBS through the lens of these infants.
