Introduction:
Individuals with multiple sclerosis (IwMS) frequently suffer from bladder disorders, and intermittent self-catheterization (ISC) is often part of management. The aim of this study was to determine the main reason for ISC introduction, and assess the outcome of this management at short and long term.
Methodology:
A retrospective study included all IwMS who learned ISC between 2018 and 2024. Data were collected upon ISC introduction (T0), 6-12 months later (T1), and at the last follow-up (T2).
Results:
One hundred fifty-nine participants were classified according to the reason for introducing ISC: 23 in the upper urinary tract (UUT) protection group, 26 in the urinary tract infections (UTIs) group, and 110 in the symptoms management group. In the symptoms management group, significant improvements were observed in overactive bladder (OAB) symptoms: nocturia decreased from mean 1.6 (sd 1.5) episodes per night at T0 to 0.5 (0.9) at T1 (p < 10- 6), and 0.2 (0.6) at T2 (p < 10- 8). In the UTIs group, the number of UTIs per year decreased over time, from 5.2 (3.9) at T0 to 1.4 (1.7) per year at T2 (p = 0.009). In the UUT protection group, 75% of participants had resolution or improvement of vesicoureteral reflux or pyelocaliceal dilations.
Conclusion:
ISC are mainly initiated to improve OAB symptoms in IwMS, and allow improvement of urinary symptoms, infections, and UUT complications, in association with detrusor overactivity management.
