When T cells with CD4 markers are activated, they give rise to two types of effector cells: helper T cells and regulatory T cells. Interleukin-17 (IL-17) pathway inhibitors show potential for treating severe localized scleroderma (LSc). While not definitive, these treatments offered improvement or stabilization in most patients refractory to traditional therapies.

  • Institutional approval was obtained for off-label drug use.
  • Main Results:

    • Five of nine patients showed improvement in their LSc.
    • Two patients experienced disease stabilization, while two patients showed progression.
    • One patient developed pustulotic arthro-osteitis, a potential paradoxical reaction; no other major adverse events were noted.

    Conclusions:

    • IL-17 pathway inhibition may offer a therapeutic option for selected patients with refractory localized scleroderma.
    • Further controlled studies are warranted to establish the efficacy and safety of these biologics in LSc.
    • These findings suggest IL-17 blockade is a promising avenue for LSc research.
