Summary
Gastric peritoneal metastases (PM) management recommendations were established using a consensus exercise. Cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) are recommended for selected patients with gastric PM.
Area of Science:
- Gastroenterology
- Surgical Oncology
- Clinical Research
Background:
- Gastric peritoneal metastases (PM) present limited evidence for management strategies.
- A consensus exercise was conducted to address these management challenges.
Purpose of the Study:
- To provide evidence-based recommendations for managing gastric PM.
- To establish consensus on key aspects of gastric PM treatment.
Main Methods:
- Modified Delphi technique employed for consensus building.
- Twenty-four questions across 8 topics were addressed by 43 experts.
- Consensus defined as >70% agreement on an option.
Main Results:
- Consensus achieved on 20/24 (83.3%) questions.
- Cytoreductive surgery (CRS) recommended for selected patients (PCI ≤ 6) in specialized centers.
- Hyperthermic intraperitoneal chemotherapy (HIPEC) recommended post-CRS for CC-0 resections, including select signet ring cell cancers.
- Prophylactic HIPEC recommended only within clinical trials.
- Systemic chemotherapy alone recommended for newly-diagnosed PM; PIPAC showed lower consensus.
Conclusions:
- CRS and HIPEC are recommended for well-selected gastric PM patients, including signet ring cell types.
- Intraperitoneal treatments for prophylaxis and newly-diagnosed PM are not recommended outside clinical trials.
- These recommendations offer guidance for clinicians managing gastric PM.
