Pseudo-Broad Ligament Leiomyoma: A Rare Cause of Diagnostic Uncertainty

Broad ligament fibroids are rare extrauterine leiomyomas that present diagnostic and surgical challenges due to their atypical location and distortion of pelvic anatomy. Large tumors may closely mimic adnexal masses, making accurate preoperative diagnosis difficult. A 44-year-old multiparous woman presented with progressive abdominal distension and a sensation of heaviness and pain in the lower abdomen for one year. Clinical examination and ultrasonography suggested a uterine fibroid, but the precise site of origin could not be determined. Intraoperatively, a large abdominopelvic mass measuring approximately 121 mm × 117 mm was identified arising from the uterine wall and extending into the broad ligament, consistent with a pseudo-broad ligament fibroid. The mass caused significant pelvic distortion with lateral displacement of the ureter. Tumor excision with total hysterectomy and bilateral salpingectomy was performed. Histopathology confirmed leiomyoma without malignancy. The postoperative course was uneventful. Pseudo-broad ligament fibroids are uncommon and often diagnosed only at surgery. Awareness and meticulous surgical technique are essential to prevent complications.
