The urology workforce is aging, with a median provider age of 54 years and approximately one-third over the age of 65. With growing workforce shortages and increasing age-related urologic disease, the need for urologists will continue to increase. Although initiatives such has residency expansion, increased use of advanced practice providers, and alternative certification pathways are being implemented, continued contributions and engagement from senior urologists is becoming increasingly important. These "Seniorologists" can offer refined clinical judgement, mentorship, communication skills, professionalism, and fundamentals of medicine such as the physical exam. Although changes in cognitive and physical performance may limit some "Seniorologists", these changes vary widely and each individual should be considered on an individual basis. Self-monitoring, ongoing education, objective evaluation, and appropriate scope of clinical responsibilities can be implemented for quality and patient safety. Capable "Seniorologists" should be supported by institutions while allowing for flexibility in workload and practice scope. As clinical responsibilities are reduced, "Seniorologists" can contribute to the field of urology through teaching, mentoring, and leadership. This type of support benefits the trainees and colleagues, the patients, and the field of urology as a whole. In an era of growing workforce shortages despite growing demands, experienced "Seniorologists" remain an invaluable asset whose continued engagement will help shape the future of urology and change patient care.
