Introduction:
Migraine is a prevalent neurovascular disorder with complex neural and vascular mechanisms. Botulinum toxin type A (BoNT-A) is an established preventive treatment for chronic migraine, yet its vascular effects remain unclear. This study investigated whether targeted BoNT-A injections along migraine-related arteries reduce headache intensity and alter vascular diameter.
Materials And Methods:
In this prospective, randomized, double-blind, controlled trial, 23 adults with chronic migraine received BoNT-A injections on one side of the head and saline on the contralateral side. Three injection points were placed along the superficial temporal artery (STA). Ultrasound measurements of the STA (proximal and distal) and supraorbital (SO) arteries were obtained at baseline, 1 h, 2 weeks, and 2 months. Pain intensity was assessed using a 10-point Visual Analogue Scale (VAS).
Results:
Mean VAS scores on the BoNT-A-treated side decreased significantly from 8.7 ± 1.46 at baseline to 2.65 ± 1.15 at 2 months (p < 0.001). No significant vascular diameter changes were detected in the STA or SO arteries between treatment and control sides (p < 0.05). No adverse events were reported.
Conclusion:
Artery-targeted BoNT-A injections produced a significant reduction in migraine pain without measurable arterial dilation, suggesting that therapeutic effects are mediated primarily through neural and neurochemical modulation rather than vascular diameter changes. This low-dose, artery-focused approach may represent a safe, efficient refinement of current migraine injection protocols.
