Abstract:
This article examines contemporary approaches to ventral and complex abdominal wall hernia repair, emphasizing preoperative optimization, risk assessment, and individualized surgical decision making. It reviews modifiable risk factors such as obesity, diabetes, smoking, and cirrhosis, and the limited evidence supporting prehabilitation. Minimally invasive mesh techniques are compared by suitability, alongside detailed operative descriptions. Component separation methods, especially posterior component separation techniques with transversus abdominis release, are contrasted for their effect on tension, recurrence, and wound morbidity. Specialized conditions are addressed. The article concludes that despite rapid robotic adoption, evidence remains low quality, making case-based selection essential.
