Abstract:
Modern dental sedation is transitioning from traditional long-acting agents toward fast-acting pharmacology characterized by organ-independent metabolism and rapid offset. In this article, we review the pharmacodynamics of inhalational agents (nitrous oxide, sevoflurane) and comprehensively evaluate novel intravenous agents: remimazolam, remifentanil, and fospropofol. Many practitioners are looking beyond traditional propofol, prompted by its narrow safety margins and the intensive airway training required to use it responsibly in a dental setting. We analyze mechanism-specific advantages, dosing protocols, and complication management. Furthermore, we detail advanced clinical strategies, including sequential benzodiazepine sedation and multi-modal total intravenous anesthesia (TIVA), equipping the oral and maxillofacial surgeon with evidence-based protocols to optimize hemodynamic stability and recovery efficiency in the outpatient setting.
