Juvenile Nasopharyngeal Angiofibroma: Maxillary Swing Approach

Introduction:
The maxillary swing approach is a very efficient surgical procedure for treating advanced-stage juvenile nasopharyngeal angiofibroma (JNA). It allows wide access to the pterygopalatine fossa, infratemporal fossa, and skull base, allowing for the entire removal of malignancies with considerable local extension.
Materials And Methods:
The purpose of this study was to assess the efficacy and outcomes of the maxillary swing technique for the treatment of extensive nasopharyngeal angiofibroma. An observational study on the presenting features, surgical technique, outcomes, and complications of the maxillary swing approach in advanced-stage JNA is presented.
Result:
A total of 11 patients treated with the maxillary swing approach were included in the study, with 6 classified as Radkowski stage IIIA and 5 as stage IIIB. There were no perioperative mortalities, and all patients were monitored postoperatively through nasal endoscopy and imaging.
Conclusion:
The maxillary swing method, a version of the trans-maxillary approach, allows for wider access to the nasopharynx, pterygopalatine fossa, and pterygomaxillary fissure. It provides a safe and effective route for approaching extensive JNAs, thereby reducing morbidity and the risk of recurrence.
