Abstract:
Hypertension affects over 1.28 billion adults globally and remains a critical contributor to cardiovascular complications, mortality, and cognitive decline. Its prevalence approaches 25% in Pakistan, with many cases remaining undiagnosed or uncontrolled. Chronic elevation in blood pressure damages cerebral vasculature, disrupts the blood-brain barrier, and accelerates white matter degeneration, leading to impairments in attention, memory, and executive functions, enhancing dementia risk. This commentary synthesises the current evidence linking hypertension-related cerebrovascular pathology with cognitive deterioration, exploring mind-motor mat training (MMMT) as a potential early, non-pharmacological adjunct to antihypertensive therapy. It integrates structured stepping sequences with concurrent cognitive challenges-such as memory recall, pattern recognition, and decision-making-to enhance neurovascular coupling, neuroplasticity, and functional balance. Based on the reviewed literature, this practice proposal supports integrating MMMT into routine hypertension management to promote cognitive preservation, reduce fall risk, and improve overall quality of life. Such an approach extends hypertension care beyond pharmacological control toward comprehensive neuroprotective and rehabilitative strategies.
