Abstract:
Evidence-based surgery depends on the deliberate generation of prospective comparative data, yet randomized trials remain underused in surgical fields because of physician preference, patient acceptance, and logistical barriers. This manuscript examines the role of clinical equipoise, skepticism, and prospective study design in challenging surgical dogma and safely evaluating innovation. Examining specific examples wherein prospective randomized trials, observational studies, and long-term follow-up investigations have formed the current best evidence for empyema management, fundoplication technique, perforated appendicitis, nonoperative management of uncomplicated appendicitis, and cryoablation for pectus excavatum repair. These examples are framed within the broader ethical and scientific obligations surrounding surgical innovation. Surgical progress requires the discipline to test assumptions, the humility to abandon favored practices when data contradict them, and the judgment to adapt study design to the magnitude and nature of innovation. Remaining curious, skeptical, and willing to learn from unexpected results is essential to preventing today's innovation from becoming tomorrow's example of avoidable harm.
