Abstract:
Theranostics pairs molecular imaging with a therapeutic radiopharmaceutical directed at the same target, with PET assessing target expression, disease extent, interlesional heterogeneity, and FDG discordance. Three established applications are reviewed: PSMA PET-guided selection for lutetium-177-PSMA-617 in metastatic androgen pathway modulation-resistant prostate cancer, somatostatin receptor PET-guided peptide receptor radionuclide therapy in neuroendocrine tumors, and iodine-124/FDG PET within radioiodine therapy for differentiated thyroid cancer. Practical guidance covers patient selection, image interpretation, treatment planning, response assessment, and reporting. A key paradigm: diagnostic uptake confirms target availability but neither measures delivered dose nor guarantees response; biomarkers, dosimetry, and response frameworks remain variably validated.
