    • This condition is rare, with limited reported cases in the Japanese literature.

    Observation:

    • An 82-year-old male presented with a significantly enlarged prostate gland.
    • The resected prostate adenoma weighed 270 grams.

    Findings:

    • Pathological examination revealed adenomatous hyperplasia of the prostate.
    • This case represents the sixth heaviest giant prostatic hypertrophy reported in Japan.

    Implications:

    • Highlights the successful surgical management of extremely large prostate adenomas.
    • Contributes to the understanding and documentation of rare cases of giant prostatic hypertrophy.
