Background:

Wilms' tumor is a common pediatric kidney cancer.
Hypertension is frequently observed in children with Wilms' tumor.

Observation:

A 4-year-old boy presented with Wilms' tumor, hypertension, and hyperreninemia.
Renin levels in tumor tissue were three times higher than in adjacent renal parenchyma.

Findings:

The Wilms' tumor was identified as the primary source of renin overproduction.
Tumor-induced renin hypersecretion is a likely cause of hypertension in this case.

Implications:

This case highlights the potential for Wilms' tumors to autonomously produce renin.
Further investigation into renin hypersecretion may be warranted in pediatric hypertension cases associated with Wilms' tumor.
