    Background:

    • Myasthenia gravis (MG) is a neuromuscular disease causing muscle weakness.
    • Ventilatory failure is a severe complication of MG.
    • Understanding factors influencing outcomes in MG-related ventilatory failure is crucial.

    Observation:

    • A retrospective study analyzed 31 patients with myasthenia gravis and ventilatory failure from 1969-1978.
    • An unusual case of chronic alveolar hypoventilation was detailed.
    • Patient outcomes were correlated with age and thymus gland status.

    Findings:

    • Younger patients with a hyperplastic thymus showed more favorable outcomes.
    • Older patients with an atrophic thymus gland had a poorer prognosis.
    • The overall mortality rate was 36%, a significant decrease from the 70% reported previously.

    Implications:

    • Thymus gland status and patient age are important prognostic indicators in myasthenia gravis.
    • Improved management strategies have led to better survival rates for MG patients with ventilatory failure.
    • Further research into the role of the thymus in MG pathogenesis is warranted.
